Two separate fees — two separate recipients. The clinic's consultation fee is paid at booking. The $150 ADHS state fee is paid separately and directly to ADHS through the ADHS Medical Marijuana Registry. The clinic does not collect any portion of the state fee. A $75 financial hardship fee is available for qualifying applicants.

Once approved, your patient card is valid for one year at any licensed Arizona dispensary statewide. Purchase up to 2.5 oz per 14-day period.
